Introduction: In recent years, the importance of community health has increased, particularly in vulnerable contexts. Methodology: A mixed-method approach was used, with a descriptive, cross-sectional, and prospective design. The sample included 385 residents of parishes identified as vulnerable, selected through cluster sampling. Data were collected using a purpose-designed questionnaire validated by specialists. Results: Of the sample, 29.9% were unemployed, and 43.1% had secondary education. The main risk factors identified were unhealthy diets (29.9%), sedentary lifestyles (29.6%), and smoking (27.3%). The most prevalent chronic diseases were hypertension (33.2%) and type II diabetes (15.8%). A total of 25.5% of the population rated air quality as poor, and 16.1% perceived drinking water as inadequate. Regarding lifestyle, 10.1% reported physical inactivity, and 28.3% had a diet of ultra-processed foods. Satisfaction with access to healthcare services was low, with 61.3% dissatisfied or very dissatisfied. Conclusions: The study highlights the high prevalence of risk factors, chronic diseases, and environmental issues in these communities, emphasizing the need for comprehensive interventions to improve socioeconomic conditions, healthcare services, and the environmental setting.
