Introduction: This research aimed to develop an instrument adapted to the Ecuadorian context to measure patient safety practices in healthcare institutions. The lack of specific and validated tools for this environment motivated the creation of a questionnaire that could accurately reflect the particularities of the Ecuadorian healthcare system. Methodology: The questionnaire was developed based on a detailed analysis of international studies on patient safety, which emphasize the importance of factors such as teamwork, effective communication, and organizational support in improving medical care. Results: The questionnaire covered six key dimensions: teamwork, effective communication, organizational support, error management, safety climate, and working conditions. These dimensions were assessed through a sample of 350 nursing students using a 5-point Likert scale. Discussion: The results showed that the questionnaire has high internal consistency, with a Cronbach's Alpha coefficient of 0.92, supporting its reliability in measuring safety practices. The instrument created will not only allow for the evaluation of patient safety in Ecuador but also contribute to the implementation of improvements tailored to the local healthcare system's needs.
